Strange occurrence testing out new 6.5 barrel

So had some extra time today and was running some reloads through my new 6.5 shilen barrel along with my 223 from my other post. But anyways using barnes 120gr ttsx which my 308 loved and this gun shoots them just a bit under 1 moa. But shot a 3 round group and heard one odd them hit the box. After my 3 shots went and took a look and the shot that I heard hit had tumbled. Shooting varget mid strength load over cci primers. No real obvious over pressure signs, only difference I noticed was the primer strike was a little deeper so maybe it was a fluke thing. Ive shot 10 more rounds through it with no other hiccups. Just wondering if maybe something I needed to check or take a look at. I individually powder charge each one so it wasn't an over throw.

Odd thing to happen for sure.
Maybe a damaged base on the bullet or something?
Sure it tumbled and didn't just rip the paper?

No I heard it hit the box and it left a hole in the box where it hit almost perfectly parallel to the box. Never had it happen before.
Thanks for any input or advice that is given.

If it doesn't happen again, it was probably a bad base on the bullet.
all bullets are steered by the base, and a ding in one corner of the base can cause them to tumble
