I can see two possible causes. Number one could be not flailing the case mouth before sweating the bullet and the other is crimping too soon.
I run everything through the Lee FCD. As has already been said I really like the carbide ring on the pistol dies because it does a final resize on finished cases.
Based on the information I got, it was more than likely a willful disregard. Someone e thought it would be funny to load some rounds that would 'surprise' his buddy.

If I was going to be doing a lot of crimped brass the Dillon Super Swage would be my choice.
I carry one as a back up. Nice little shooter, for what it is. You will never mistake it for a J frame Smith but it is a good gun. I agree with Sawman, no +P needed.

Take a look at bambamholsters.com out of Amarillo. I carry a 1911 and a J frame Smith both in his Avenger model.
I would try both and maybe one of the Nosler offerings as well. All three are quality hunting projectiles but only some experimenting will tell you which is "the best" in your gun.
I am 5'3" and my bench is 42". It works out perfect for me either standing or with a bar stool.
